In people with Alzheimer's disease (AD) the worsening of symptoms such as cognitive, functional, and behavioral disorders make them completely dependent, thus establishing the need for a caregiver. Objective: To verify the greatest difficulties faced in the daily lives of caregivers of patients with Alzheimer's disease. Method: Conducted through a systematic review of original articles, from 2016 to 2021. Articles published and indexed in MEDLINE (Online System of Search and Analysis of Medical Literature) were used in the search platforms PUBMED and SciELO (Scientific Electronic Library Online), as inclusion criteria addressing the objective proposed by the research, published between 2016 and 2021. Results: After the thematic inclusion criteria, 116 articles were found in SciElo and 1675 in PUBMED. After methodological design criteria and exclusion criteria, 9 articles from SciElo and 4 from PUBMED were selected to compose the systematic review of the literature. Discussion: Among the main difficulties experienced by caregivers, we highlight: the need to learn about the disease; dealing with guilt; face situations of pain, dependence and physical and psychological suffering. In this scenario, caring for a family member with AD requires the caregiver to learn to live with the suffering of the other, hiding their own pain and needs. Conclusion: The main difficulties of caregivers of people with Alzheimer's are the suffering with the workload and the emotional and psychological pain of caring for a person who will become increasingly dependent.
