Objective: To elucidate the main mechanisms of orthopedic trauma with fractures in patients aged 0 to 12 years treated at the pediatric emergency department in a hospital in western Paraná. Method: A quantitative epidemiological, observational, descriptive, retrospective study was conducted based on data collection and analysis of medical records (352) of patients seen at the pediatric emergency department between January 2019 and December 2020. Patients who did not obtain a diagnosis of fracture (193) were excluded from the analysis. Results: From the sample of 159 analyzed medical records, the prevalent biological sex was male (52.5%). Regarding age, the most relevant age range was between 2 and 5 years (49.3%), with a peak of 5 years (15.8%). Regarding fracture classification, 71.4% were complete and 82.3% were caused by direct trauma, with the upper limbs being the most affected (84.8%), predominantly in the left hemibody (55.7%). The most commonly affected bone was the radius (39.2%) and the most frequent trauma mechanism was a fall from the same level (48.5%), with bicycle falls being the most relevant (10.1%). Secondarily, the fall from elevated level was significant, characterizing 32.2% of the cases, of which the fall from bed was the most prevalent, portraying 23.1%. Moreover, regarding the choice of therapeutic method, the conversational treatment was prevalent in 75% of the occurrences. Conclusion: Studies such as this present important information for preventive measures for children, through health promotion policies and prevention of accidents that cause damage or sequelae in this population.
