Investigación de genes de resistencia a aminoglucósidos en aislados clínicos de Acinetobacter spp. en un hospital de Recife - PE

Resumen

Los aminoglucósidos son una de las opciones terapéuticas utilizadas en el tratamiento de infecciones causadas por bacilos gramnegativos multirresistentes, incluidas las cepas de Acinetobacter baumannii. Esta especie bacteriana es considerada un patógeno oportunista que logra desarrollar resistencia a los aminoglucósidos a través de la degradación enzimática por enzimas modificadoras de aminoglucósidos (AME) y metilasas de rRNA 16S. El presente estudio evaluó en aislados clínicos de Acinetobacter spp resistentes a aminoglucósidos de un hospital de Recife-PE, la ocurrencia de AME y metilasas, además de evaluar la relación clonal entre los aislados. Se investigaron AME (aac (6 ') - Ib, hormiga (2 ") - Ia y hormiga (3") - Ia) y metilasas (armA, rmtB, rmtC, rmtD, rmtF, rmtG) en 35 aislamientos de Acinetobacter. Spp. resistentes a uno o más aminoglucósidos, y se realizó ERIC-PCR para determinar la relación clonal. De los tres genes AME estudiados, ant (3 ') - Ia fue el gen más prevalente presente en el 43% (15/35) de los aislamientos, seguido de ant (2) -Ia con menos del 1% (3/35) y no se detectó el gen aac (6 ') - Ib. Entre las metilasas, armA y rmtC fueron los genes más prevalentes, con 80% (28/35) y 57% (20/35), respectivamente. No se detectaron los genes rmtB, rmtD, rmtF y rmtG. La tipificación molecular demostró la aparición de dos clones en el hospital. Los datos obtenidos describen un avance en la resistencia a los aminoglucósidos, la ocurrencia de genes detectados con un patrón similar a otros estudios previos, una relación de clonalidad entre aislamientos de Acinetobacter spp que demuestran diversidad en el ambiente hospitalario y la necesidad de métodos para prevenir la propagación de estos clones. en hospitales de Recife, Pernambuco.
