Análisis de la distribución de tenciones en maxilares sometidos a expansión rápida asistida quirúrgicamente con un dispositivo de soporte óseo

Resumen

El proposito de este estudio fue evaluar la distribución del estrés producido por la expansión maxilar rápida asistida quirúrgicamente en las estructuras maxilares utilizando el dispositivo expansor soportado en hueso por el Método de los Elementos Finitos. Material y métodos: Se simuló un modelo tridimensional para cirugía con osteotomía LeFort I con disyunción pterigomaxilar utilizando un modelo de dispositivo de soporte óseo para la apertura de 1 mm. Los resultados mostraron una mayor apertura del maxilar en la región anterior y una menor apertura en la región posterior. Las fuerzas de tensión se manifestaron principalmente en toda la superficie palatina y el hueso alveolar posterior y tubérculo, así como en la región de pilar cigomático por encima de la osteotomía. El estrés mostró poca disipación para las raíces del molar. Las fuerzas de compresión se manifestaron en el proceso pterigoideo y el pilar cigomático adyacentes a las osteotomías, así como en las áreas de contacto del expansor con el hueso palatino. El análisis de tensión de Von Mises mostró que la parte que sufrió más tensión es la varilla que conecta el módulo del dispositivo con la plataforma de soporte. Conclusión: Los esfuerzos de tensión resultantes transmitidos al hueso alveolar de los dientes adyacentes al aparato no parecen ser suficientes para desplazar los dientes.
