Análisis de prescripciones de albúmina humana al 20% de un hospital universitario y su respectiva descripción de gastos

La albúmina es el expansor de plasma más importante en el torrente sanguíneo. En forma de medicamento, debe prescribirse después de una evaluación minuciosa de los pacientes, basando su uso de acuerdo con la literatura, ya que tiene contraindicaciones, como sangrado en pacientes posquirúrgicos, además de ser un medicamento de alto costo. Así, el objetivo de este trabajo fue realizar un levantamiento de las indicaciones terapéuticas contenidas en las prescripciones de albúmina humana (HA) 20% de un Hospital Escuela en la ciudad de Recife, Pernambuco, Brasil, con el fin de evaluar si tales indicaciones tener base científica, además de realizar un relevamiento de los costos relacionados con la adquisición de este fármaco. Se realizó un estudio transversal, retrospectivo, a partir del análisis de las recetas prescritas a los pacientes, desde mayo de 2015 hasta mayo de 2017. Las indicaciones terapéuticas encontradas fueron: paracentesis de alivio (29%), síndrome hepatorrenal (SHR) (22,9 %), hipovolemia (20,6%), shock séptico (8,7%), peritonitis bacteriana espontánea (PBE) (6,4%), hipoalbuminemia (6,1%), ascitis (4,1%), neoplasia (1,2%) y encefalopatía hepática (EH) (0,6%). Después de una revisión de la literatura, HA 20% tiene respaldo científico para el tratamiento de paracentesis, SHR y PBE, lo que corresponde al 63,1% de las prescripciones y no tiene respaldo científico para el tratamiento de hipovolemia, shock séptico, hipoalbuminemia, cáncer y HE (36,9%), que pueden ser reemplazados por otros expansores de plasma. Así, se destaca la importancia de fundamentar la prescripción de AH al 20% de acuerdo con la literatura, siempre visando el uso racional de los medicamentos.
