El papel del concentrado de complejo de protrombrina en el tratamiento de la coagulopatía inducida por trauma: una revisión integradora

Introducción: El trauma es una enfermedad que mata a casi 6 millones de personas cada año. El sangrado resultante de un traumatismo desencadena una coagulopatía compleja, que requiere un tratamiento inmediato para mejorar la supervivencia del paciente. El tratamiento se basa en la eliminación del origen del sangrado, utilizando hemoderivados y concentrados de factores de coagulación, asociados a hipotensión permisiva y reposición controlada de líquidos cristaloides. Objetivo: Evaluar los efectos del uso de concentrados de complejo de protrombina en el tratamiento de la coagulopatía inducida por traumatismos en el servicio de urgencias. Metodología: este estudio es una revisión integrativa de la literatura. El mecanismo de búsqueda utilizado dentro de las bases de datos fue: (Bleeding OR Hemorrhage OR Hemostasis OR “Blood Coagulation Disorders") AND (“Prothrombin Complex Concentrates”). Incluyó artículos de 2017 a 2022, textos completos disponibles en inglés, portugués y español. Se excluyeron cartas, editoriales, relatos de casos y duplicados. Resultados: Después de aplicar los criterios de inclusión y exclusión, se seleccionaron 37 artículos para lectura completa, resultando una muestra final de 15 artículos para componer esta revisión integradora, nueve de MEDLINE, cinco de SCOPUS y uno de SCIENCEDirect. Conclusión: El concentrado de complejo de protrombina es un coadyuvante importante en el manejo de la coagulopatía inducida por trauma, debido a la capacidad de reducir la razón internacional normalizada y la mortalidad sin aumentar el riesgo de eventos tromboembólicos.
