Evaluación de la seguridad terapéutica de los betabloqueadores en la insuficiencia cardíaca aguda en emergencia hospitalaria

Los betabloqueantes son fármacos esenciales para el tratamiento de la Insuficiencia Cardíaca (IC) Crónica, y su uso para el control de la frecuencia cardíaca (FC) en la IC aguda no fue adecuadamente estandarizado debido a la asociación con efectos adversos graves. Sin embargo, estudios recientes sugieren que el uso de estos fármacos puede reducir las tasas de morbimortalidad generadas en la IC aguda. Así, el objetivo de este estudio es evaluar la seguridad de los bloqueadores beta en pacientes hospitalizados por insuficiencia cardiaca aguda en el servicio de urgencias hospitalario. Esta es una revisión integrativa de la literatura realizada entre enero y junio de 2023 con base en las bases de datos PubMed y BVS. Al final de la selección, se analizaron 13 estudios, entre ensayos clínicos aleatorizados, estudios prospectivos, retrospectivos y ambispectivos, informes de casos y revisión sistemática, en los que se demostró la seguridad clínica de diversos betabloqueantes con una reducción significativa de la morbimortalidad, bajas tasas de efectos adversos asociados en pacientes con IC aguda y buena perfusión periférica, pero su uso sigue proscrito en pacientes con bajo perfil de gasto. Por lo tanto, los bloqueadores beta, especialmente caverdilol y landiolol, se recomiendan para el control de la FC en pacientes con IC aguda y buena perfusión, ya que reducen la morbimortalidad y los reingresos por estas condiciones.
