Fibroma ossificante juvenil psamomatoide: relato de caso

As lesões fibro-ósseas são um grupo de lesões caracterizadas pela substituição do osso normal por um tecido conjuntivo fibroso e tecido mineralizado. Dentre essas lesões está o fibroma ossificante juvenil, um tumor fibro-ósseo benigno, raro, abrangendo aproximadamente 2% dos tumores bucais que acometem crianças e adolescentes. De acordo com as características histopatológicas, é subdividido em fibroma ossificante juvenil psamomatoide e fibroma ossificante juvenil trabecular. A variante psamomatoide apresenta predileção pelo gênero masculino, idade média de 22 anos, com predominância na maxila. Geralmente esse tipo de tumor é descoberto em exame radiográfico de rotina, sendo que sua expansão cortical pode resultar em um aumento facial detectável. Seu tratamento e prognóstico são incertos. Para as lesões menores pode ser realizada a excisão local ou uma curetagem cuidadosa. Para as lesões de crescimento rápido e/ou agressivo pode ser necessária uma ressecção em bloco com margens conservadoras associada ou não ao uso de enxertos ósseos e placas de reconstrução. O objetivo do relato é descrever a condição de uma paciente de 26 anos, encaminhada ao Hospital Universitário do Oeste do Paraná com diagnóstico de fibroma ossificante juvenil psamomatoide em mandíbula. Após a realização do exame clínico e análise dos exames de imagem, foi realizado o tratamento endodôntico dos elementos 34 ao 42. O tratamento consistiu na curetagem, enucleação da lesão e apicectomia dos dentes envolvidos, evoluindo com um pós-operatório sem intercorrências. A paciente está em proservação há 01 ano e 07 meses.
