Repercussões cardiovasculares decorrentes da Síndrome Multissistêmica Inflamatória após infecção por COVID-19 em idade pediátrica: uma revisão integrativa

Resumo

A população pediátrica constitui apenas 2% dos casos de COVID-19, contudo, tem-se relatado casos de choque inflamatório, disfunção ventricular e de eventos trombóticos. Esse conjunto de sinais é denominado de Síndrome Inflamatória Multissistêmica (MIS-C), caracterizada também por hiperpirexia e falência generalizada de órgãos, havendo relação direta com a infecção pelo SARS-COV-2. O presente trabalho objetiva sintetizar as evidências das manifestações cardiovasculares decorrentes da Síndrome Multissistêmica Inflamatória pós COVID-19 em idade pediátrica (PMIS-TS). Para essa Revisão Integrativa, realizada através das das plataformas PubMed e Science Direct entre 2020 e 2022,  os descritores eleitos foram ''Inflammation'', ''Pediatrics'', ''Covid-19'' e ''Cardiology'', associados ao operador booleano ‘’AND’’. Os critérios de inclusão consistiram em (1) ensaios clínicos, testes controlados e aleatórios, revisões e meta-análises; (2) trabalhos sobre as repercussões cardiovasculares da PMIS-TS; (3) estudos sobre as cardiomiopatias pediátricas oriundas de infecção por coronavírus; (4) artigos sobre condução e tratamento da PMIS-TS. Os critérios de exclusão compreenderam inadequações ao tema, avaliações de outras afecções clínicas, além de textos inespecíficos ao abordarem a MIS-C. Observou-se que os casos de PMIS-TS detiveram ecocardiogramas e ressonâncias nucleares magnéticas preditivas de miocardite, valvulite e coronariopatias, achados corroborados por alterações laboratoriais. Os eletrocardiogramas incluíram batimentos prematuros e arritmias diversas. Na análise dos 54 artigos, identificou-se que, apesar da faixa etária pediátrica corresponder a menor parcela dos casos de COVID-19, as crianças são suscetíveis a repercussões potencialmente graves. Assim, o estado hiper-inflamatório pode conduzir ao óbito, sendo imprescindível a condução de novos estudos com opções terapêuticas assertivas para os quadros supracitados.
