Sintomas de ansiedade e depressão em pacientes com zumbido: revisão sistemática

Resumo

Objetivo: comparar portadores e não-portadores de zumbido quanto à presença de sintomas depressivos e ansiosos. Metodologia: foram realizadas buscas nas bases de dados PubMed e Lilacs de estudos com humanos adultos, do tipo transversal, coorte ou caso-controle, os quais foram selecionados em duas etapas (leitura do título e resumo e leitura do texto inteiro). A seleção, extração dos dados e avaliação da qualidade dos artigos foram realizadas por dois revisores independentes. Resultados: foram incluídos 29 artigos, abrangendo 1.208.435 participantes. 78% e 76% dos artigos encontraram associação positiva e significativa (p < 0,05) do zumbido com depressão e ansiedade, respectivamente. Todos os artigos que compararam o subgrupo de portadores de zumbido não-incômodo com os controles quanto à depressão encontraram associação positiva e significativa (p < 0,05). Todos os artigos que avaliaram a correlação do incômodo do zumbido com os níveis de ansiedade ou depressão encontraram correlação positiva e significativa (p < 0,05). Conclusão: portadores de zumbido tem maior chance de apresentar sintomas depressivos e ansiosos que não portadores, e tal associação é independente da perda auditiva. A prevalência e a intensidade de sintomas psiquiátricos são maiores quanto maior o sofrimento acarretado pelo tinido, mas zumbidos estão associados a maior chance de depressão mesmo quando não provocam incômodo.
