Protocolos de avaliação clínica da propedêutica não invasiva de dor torácica no departamento de emergência: Uma revisão sistemática

Introdução: O manejo inicial da dor torácica é uma situação desafiadora para os médicos emergencistas devido às diferentes etiologias. Para garantir este manejo efetivo foram elaborados diferentes protocolos de avaliação. Objetivo: Identificar os protocolos de avaliação clínica para a propedêutica não invasiva de dor torácica em adultos, no departamento de emergência (DE). Métodos: O estudo foi realizado de acordo com PRISMA e registrado na plataforma PROSPERO (CRD42024519268). Foi realizada uma busca sistemática abrangente nas bases de dados internacionais, incluindo o PubMed e SciELO, para encontrar artigos publicados até abril de 2024. Todos os artigos relevantes que relataram protocolos de avaliação clínica para dor torácica em adultos foram incluídos na análise. Resultados: Nesta revisão foram incluídos artigos com orientações e protocolos para o manejo de dor torácica nos DEs. Identificamos que a avaliação da dor torácica no DE envolve uma abordagem sistemática. A análise inclui história do paciente, exame físico, diagnóstico diferencial, testes diagnósticos e estratificação de risco. Os estudos ainda apontam para a importância do eletrocardiograma de 12 derivações, elevações nos níveis de troponina e os protocolos ADP, EDACS e HEART, a fim de determinar o risco de um paciente ter um evento cardíaco adverso maior. Conclusão: Este artigo destaca a importância do uso de protocolos baseados em evidências e vias de decisão na avaliação da dor torácica, no DE. Enfatiza ainda, o papel dos ensaios de troponina de alta sensibilidade e das modalidades de imagem no diagnóstico rápido e preciso de condições, como a síndrome coronariana aguda.
