The objective of this study was to investigate the cardiovascular profile of patients affected by ischemic stroke, identifying risk factors and comorbidities associated with the event and its recurrence. This is a cross-sectional study with a quantitative approach, carried out in a hospital in the Zona da Mata region of Minas Gerais, with a sample composed of patients admitted with ischemic stroke. Data collection was performed using the Stroke Prognosis Instrument II and Essen Stroke Risk Score, used to estimate the risk of recurrence or death based on the identification of risk factors. The results showed that the sample was predominantly composed of men aged 60 years or older, with only one individual not presenting any of the risk factors assessed by both instruments. Regarding risk stratification, it was found that, according to the Stroke Prognosis Instrument II, there was a predominance of moderate risk, followed by low risk, with no record of patients classified as high risk. In contrast, the application of the Essen Stroke Risk Score revealed a distinct distribution, with a higher proportion of patients classified as high risk and the remainder as low risk, considering the absence of a moderate risk category in this instrument. It is concluded that the patients evaluated present a high burden of cardiovascular risk factors, and that, although both instruments used are useful tools in prognostic stratification, their methodological differences influence risk classification and should be considered in clinical interpretation and in the planning of strategies for preventing the recurrence of ischemic stroke.
