Serial dental extractions and immediate complete denture installation in a patient with chronic kidney disease and stage IV periodontitis: Case report

Chronic Kidney Disease (CKD) is a progressive condition associated with persistent systemic inflammation and high morbidity and mortality, in which the presence of oral infectious foci, such as Periodontal Disease (PD), can worsen the clinical picture and compromise the patient's stability. This study aims to describe a clinical case of a patient with CKD, presenting with stage IV periodontitis, who underwent serial extractions and placement of Immediate Complete Dentures (ICD) in the upper and lower jaws. A 50-year-old male patient with CKD associated with systemic arterial hypertension presented with generalized tooth mobility, advanced attachment loss, a mean probing depth of 7 mm, occlusal collapse, and impaired masticatory function. The treatment plan consisted of oral environment preparation through scaling and root planing, followed by minimally traumatic serial extractions of the remaining teeth. Prior to the surgical procedure, prosthetic planning was performed with the fabrication of mucosally supported immediate complete dentures. The extractions were performed under local anesthesia, followed by alveolar curettage, suturing, and immediate placement of prostheses, which acted as healing matrices. The pharmacological protocol included antibiotic therapy with azithromycin and analgesia with dipyrone. At the six-month follow-up, adequate healing, absence of complications, and restoration of masticatory function, vertical dimension of occlusion, and facial aesthetics were observed, with a significant improvement in quality of life. It is concluded that the integrated approach was effective, safe, and fundamental for functional rehabilitation and inflammatory control in a patient with CKD.
