Hemodialysis is considered one of the most prevalent life-sustaining renal replacement therapies in Brazil. Despite its essential nature, this treatment imposes significant changes on the daily lives and occupational performance of individuals. Therefore, this study aims to describe the occupational performance and satisfaction based on the self-perception of patients undergoing hemodialysis treatment. This is an observational, cross-sectional study with a quantitative approach, conducted at a University Hospital in Pernambuco. The Canadian Occupational Performance Measure (COPM) was used to assess occupational performance. Sociodemographic and occupational performance data were tabulated and analyzed using frequency comparison tests (chi-square) and mean comparison tests (Student's t-test and ANOVA), using Microsoft Excel and SPSS software. The main complaints reported by participants focused on the areas of self-care and productivity, especially related to functional mobility, household chores, and work activities. Fewer complaints were observed in the Leisure area, but with low levels of occupational performance and satisfaction. Aspects related to impairments in significant occupational roles for individuals are noticeable, as well as a tendency towards modifications in activity performance with adaptations to maintain active participation, and also a reduction or abandonment of their occupational roles. The impairments resulting from hemodialysis treatment interfere with performance and satisfaction in human occupations, due to the limitations imposed by the therapeutic routine, physical symptoms, and insecurities related to the use of vascular access. This highlights the relevance of Occupational Therapy in this care context, given the occupational repercussions.
