Analysis of depression and depressive symptom management in Primary Health Care in a municipality in the far south of Santa Catarina

Brazil has the highest prevalence of depression in Latin America, with rates of 5.8%, impacting public health significantly. Depressive disorders should be identified and treated within Primary Care – the entry point to Brazil's healthcare system. However, despite this importance, there is a limited amount of literature addressing its management in the context of Primary Care, particularly in Brazil. This study aims to investigate the management of depression and depressive symptoms in Primary Health Care in a city in the far south of Santa Catarina. It is a cross-sectional, descriptive study based on data collection. Included in the study were medical professionals working in the city’s Basic Health Units during data collection. Excluded were professionals who were absent or who did not consent to participate. The results showed that the final sample profile primarily consisted of recent graduates, men, who attended at least 15 consultations per day. The initial management of depression is performed regardless of the main complaint of the consultation, with a comprehensive evaluation, with insomnia being the most prevalent symptom in diagnostic suspicion. Risk stratification is carried out by most, based on clinical assessment. The most commonly adopted therapy involves the pharmacological class of Selective Serotonin Reuptake Inhibitors, physical activity, and psychotherapy. The main limitations identified for managing depression in public Primary Health Care were the limited availability of free pharmacological options and delays in access to specialized care.
