Abstract

Orthognathic surgery plays a key role in the correction of dentofacial deformities, promoting significant improvements in facial aesthetics, masticatory function, and patients’ quality of life. However, complications such as fixation plate fractures may occur, even in cases with appropriate surgical planning and meticulous technical execution. This article aims to report the case of a patient who underwent orthognathic surgery and presented with bilateral fracture of the titanium plates in the early postoperative period, associated with previously undiagnosed bruxism. Management included surgical reintervention and the administration of botulinum toxin as a complementary approach, aiming to reduce deleterious muscular forces and prevent recurrence. The discussion highlights the biomechanical and physiological factors involved in such complications, as well as the need for thorough preoperative assessment and multidisciplinary strategies to mitigate risks. This case underscores the importance of identifying and managing parafunctional habits and highlights the role of botulinum toxin in the context of orthognathic surgery, contributing to more stable clinical outcomes and reduced impact of postoperative complications.
