Clinical, radiographic, and tomographic analysis of endodontic retreatment in molars – case series

Despite advances in the field of Endodontics, there are still cases that result in failure, making further intervention necessary. In this context, conventional endodontic retreatment is the first-choice alternative, offering a good prognosis when indications and limitations are respected. These therapy failures can be acquired through several iatrogenic factors, which may be pathological, technical, and/or anatomical. Therefore, the objective of this work was to discuss endodontic retreatment in molars, analyzing success and failure through the report of a series of clinical cases. Digital dental records, radiographic images, and tomographies of four patients treated at the Endomais clinic, located in the city of Uberlândia in the state of Minas Gerais, were analyzed. All retreatment cases were female patients, aged between 18 and 62 years, two in upper molars and two in lower molars. The four cases presented periapical lesions and underfilled canals; in two of them, there was clinical presence of fistula, and in two cases, there was root perforation caused by the first treatment. After 12 months of follow-up, 3 were successfully retreated. This study highlights that endodontic retreatment in molars is a very complex therapy that requires knowledge of dental anatomy and physiology, the use of certain technologies, and, above all, a specialist in Endodontics to perform it. Among the technologies, the use of computed tomography was extremely important and indispensable for the therapy.
