Orofacial harmonization represents an advancement, integrating oral health and aesthetics with techniques such as fillers, botulinum toxin, and procedures for delicate areas like the lower eyelids. The use of materials such as hyaluronic acid and PMMA allows for natural, personalized, and long-lasting results, being fundamental for correcting imperfections and promoting facial rejuvenation. Qualified professionals play a crucial role in ensuring safety and efficacy, while methods such as collagen stimulation complement the treatments. The objective of this study is to analyze the use of PMMA in the lower eyelids, with emphasis on its aesthetic application and safety. The search was conducted in the PubMed, SciELO, Medline, and LILACS databases, using the descriptors "PMMA," "Biocompatibility," and "Harmonization." Articles in Portuguese and English were included. Duplicates and texts without full access were excluded. The results showed that PMMA is used due to its biocompatibility, durability, and efficacy in aesthetic and medical applications. In facial harmonization, it offers a permanent solution for filling wrinkles and lipoatrophy, especially in HIV patients, demonstrating high efficacy and long-lasting results. However, complications such as granulomas, necrosis, and inflammatory reactions can occur, making the choice of high-quality products, appropriate techniques, and the professional's experience essential. Despite its aesthetic and controversial potential, the use of PMMA in the lower eyelid lacks robust evidence and long-term studies, indicating the need for more research to validate its safety and efficacy in this delicate area.
