Preparation of a pharmacotherapeutic follow-up form for patients using tamoxifen

Objective: To develop a pharmacotherapeutic form for the pharmaceutical monitoring of patients who use tamoxifen during breast cancer treatment, aiming to identify drug interactions, Drug-Related Problems (DRPs) and improve treatment adherence. Methodology: This is a methodological, prospective study of a documentary nature, aiming to develop an instrument for the use of pharmacists in pharmacotherapeutic monitoring within the scope of clinical pharmacy. The form was prepared based on the Dáder Method of pharmacotherapeutic monitoring, with modifications aimed at focusing on monitoring adverse reactions, problems related to the medication and adherence, whose evaluation was based on the Morisky Green test, which consists of four questions with dichotomous answers and assigning a score according to the answer. Results: The pharmacotherapeutic form was designed to enable pharmaceutical monitoring of cancer patients with the purpose of observing their needs and ensuring that this pharmacotherapy is carried out without interference. In the first section of the form, spaces are included to fill in the patient's basic data. Section 2 was reserved to allow the patient to report how she handles tamoxifen treatment. The Morisky Green test will also be applied to assess adherence. Conclusion: The development of a pharmacotherapeutic monitoring form will help the pharmacist to know the patient's health conditions, since cancer patients who use oral medications and have other pathologies must be monitored continuously to ensure treatment is carried out in an efficient manner, correct and safe.
