The aim of this study was to report a case of type I naso-orbito-ethmoidal (NOE) fracture, emphasizing diagnostic aspects, surgical management, and outcomes. This observational case report was based on clinical evaluation and imaging exams, including computed tomography with three-dimensional reconstruction. The patient presented with minimal displacement, preservation of the medial canthal tendon, and absence of significant comminution. Open reduction and rigid internal fixation were performed at three strategic anatomical points, followed by suturing and postoperative follow-up. The evolution was satisfactory, with no significant functional or aesthetic complications. This case highlights the importance of early diagnosis, accurate classification according to Markowitz and Manson, and the application of appropriate surgical techniques to restore facial anatomy and function. Careful management of these injuries can prevent severe sequelae, such as persistent telecanthus and nasolacrimal system dysfunction, ensuring better patient outcomes.
