Cardiac tamponade secondary to myocarditis caused by Coxsackie B

Viral myocarditis is an inflammatory condition that can range from subclinical forms to severe presentations, such as fulminant heart failure and cardiac tamponade. The association with endocrine dysfunctions, such as congenital panhypopituitarism and central hypothyroidism, is rare and scarcely described in the literature. The present work aims to report the case of a young patient with congenital panhypopituitarism and midline malformation who developed cardiac tamponade resulting from viral myocarditis caused by Coxsackievirus B. The patient underwent emergency median sternotomy, with removal of a large volume of pericardial fluid, subsequently evolving with severe myocardial dysfunction and reduced ejection fraction. A review of the literature demonstrates that hormonal deficiency may contribute to the greater severity of the inflammatory response and predispose to severe cardiovascular complications. This report reinforces the importance of the early recognition of cardiovascular manifestations in patients with endocrine diseases, as well as the role of a multidisciplinary approach in the management of complex cases.
