Hipercalcemia PTH-independente associada a granulomatose induzida por óleo mineral: Relato de caso

Resumo

A injeção de óleo mineral para fins de aprimoramento estético é uma preocupação de saúde pública pois pode desencadear respostas inflamatórias crônicas e a formação de granulomas. A inflamação granulomatosa pode levar à hipercalcemia PTH-independente por meio da produção ectópica de 1,25-di-hidroxivitamina D (calcitriol) por macrófagos ativados. A pesquisa tem o objetivo de apresentar um estudo de caso clínico envolvendo um homem de 42 anos com histórico de injeções intramusculares de óleo mineral que se apresentou com hipercalcemia grave, nefrocalcinose, nefrolitíase e doença renal crônica. Exames laboratoriais revelaram cálcio e fosfato séricos elevados, paratormônio (PTH) suprimido, 25-hidroxivitamina D baixa e calcitriol marcadamente elevado. O paciente respondeu bem ao tratamento com prednisona oral e hemodiálise, resultando na normalização dos níveis de cálcio. Este caso enfatiza a necessidade de considerar a granulomatose induzida por óleo mineral como uma causa potencial de hipercalcemia não dependente de PTH, especialmente em pacientes com histórico de procedimentos estéticos que envolvem substâncias à base de óleo.
