Frequency and characterization study of Klebsiella pneumoniae carbapenemase infections in a hospital in the Campos Gerais region

Aim: To determine the profile and prevalence of Klebsiella pneumoniae carbapenemase-related infections in health care. Method: This was a retrospective cross-sectional descriptive study (Yin, 2015) and analyzed the profile of these infections in a single public hospital between 2019 and 2021, through data collection with the Center for Hospital Infection Control and in electronic medical records. The data collected included demographic, clinical, and laboratory information related to infection. Results: Fifty-nine patients with KPC-associated infection were analyzed (8.3% 65/780), with Klebsiella pneumoniae being the most isolated species (93.4%, 57/61). The highest number of infections was found in the intensive care unit (76.7%, 46/60), most related to ventilator-associated pneumonia (29.2%, 19/65), the presence of 3 comorbidities or more (55.9%, 33/59) and assisted with at least one invasive device (78%, 46/59). Prior antibiotic therapy was found in 96.7% (58/60) and, after culture, 31.7% received amikacin and polymyxin B. Of these patients, 66.1% (39/59) evolved to death and 33.9% (20/59) received medical release. Conclusion: the results agree with the current literature, highlighting the importance of preventive measures to the dissemination of multidrug-resistant agents and the impact of the COVID-19 pandemic on the increase of HCAIs by Gram-negative bacteria.
