Erosive tooth wear associated with Pica syndrome: clinical case report

The purpose of this qualitative case report is to report the association of Pica Syndrome with severe generalized tooth wear through a clinical case report. A 47-year-old female patient attended complaining of discomfort and chewing difficulty in the entire upper and lower dental arch. The anamnesis revealed that the patient had the habit of geophagia, characterized by chewing and ingestion of building bricks in a period of one year and six months, in addition to the diagnosis of anxiety, depression, anxiety, bipolar disorder, iron deficiency and history of surgery bariatric. The clinical examination revealed extensive generalized dental wear, suggestive of abrasion due to the history of geophagia, on the occlusal surface of the posterior teeth, incisal and palatal of the anterior teeth. In addition to the abrasion, there were small spots of chemical erosion. The diagnosis was Dental Abrasion and Erosion associated with Pica Syndrome. The restorative dental treatment proposed, discussed and accepted was the rehabilitation of anatomy and masticatory function through the use of composite resins, due to the socioeconomic status of the patient. It is concluded that the dental surgeon must be prepared to diagnose the oral manifestations related to Pica Syndrome, as well as its oral impacts in order to provide an adequate treatment for each patient. Generalized dental abrasion and erosion are oral manifestations of Pica Syndrome associated with geophagia and may be associated with female individuals with depression, anxiety, bipolar disorder, iron deficiency and a history of bariatric surgery.
