Electroconvulsive therapy (ECT), as an effective, safe, internationally recognized and accepted therapeutic method, must be performed under appropriate conditions. This article will present a descriptive and historical report on the implementation of the electroconvulsive therapy procedure in the State of Tocantins, including its difficulties and successes. For this, a qualitative and descriptive research was carried out. Resistance was observed in health teams and managers due to stigma and lack of knowledge. However, guidance and good results made it possible to overcome barriers and make the procedure accepted and demanded. For the future, it is expected to expand neuromodulation activities, either through more indications and a greater number of technical applications.
