Context: Brazil has the third largest prison population in the world, with 773,000 people incarcerated, according to the Federal Government. Objective: To present prevention strategies in a Regional Penal Unit (UPR) in the State of Tocantins. Methodology: This is a cross-sectional study, carried out in the database and internal flows for control measures of COVID-19 in the institution, in the period between March 2020 and January 2022. Results: The study shows a reduction of prisoners in the period studied with the lowest prevalence in 2021 (634; ± 45.85%). The prison population of the study has a young profile (n=328; 50.0%), black or brown (n=517; 79.0%), poor (n=318; 57.0%) and with low schooling (n=355; 53.0%). Therefore, care flows were elaborated and standardized for greater objectivity and promptness in maintaining the health of the Freedom Deprived and the servants in the UPR. Conclusion: Prisons are prone to a high risk of transmission of Covid-19, in view of their space limitations and precarious environmental and hygienic conditions. The recommendations for the control and management of SARS-CoV-2 infection should consider some peculiarities of the prison, such as: identification of symptomatic, serial testing of shared housing contacts and immediate isolation. Finally, the elaboration of educational/informative materials, as well as the standardization of care flows, is fundamental for a better resolution in the face of a new and complex situation.
