Early loss of the dental element is characterized by exfoliation before the expected time and causes the lack of functions that these teeth performed, among them the preservation of the space of permanent successors, establishment of occlusion, chewing, phonation and aesthetics. Objective: To report a clinical case of making a conventional fixed space maintainer as an orthodontic intervention for early loss of deciduous teeth and control of unilateral posterior vertical growth. Methodology: This is a case report, qualitative and descriptive. Case report: Female patient, 05 years old. After anamnesis, clinical and radiographic examinations, the extraction of element 85 was indicated as the treatment of choice and subsequent construction of a space-maintaining orthodontic device. To contain the posterior vertical growth of the maxilla on the right side, the non-functional device was adapted with acrylic resin, restoring the patient's occlusion and growth in a functional manner. Discussion: The correct position of the element in the dental arch is a consequence of the balance of forces acting on it. The disharmony between them directly alters its positioning, as well as can generate imbalance in vertical bone growth (asymmetry). Conclusion: Orthodontic intervention became indispensable and acted in a preventive and interceptive way. For both, the conventional fixed space maintainer of the loop band type was an effective alternative.
