Continuidad y transición del cuidado de niños con condiciones crónicas: una revisión del alcance

Resumen

La creciente disponibilidad de recursos tecnológicos y el cambio en el perfil nutricional de la población también han aumentado la supervivencia de los niños con enfermedades crónicas. La atención a los niños con enfermedades crónicas requiere un enfoque diferenciado y especializado, considerando sus cuidados específicos y vulnerabilidades. Este artículo tiene como objetivo comprender la evidencia encontrada en la literatura científica nacional e internacional sobre la continuidad y / o transición en la atención de niños con enfermedades crónicas. Para ello, se realizó una revisión de alcance en las bases de datos BVS, Pubmed y Scopus en enero de 2019, con una búsqueda de datos entre 2009 y 2018, utilizando los descriptores "Continuity of Patient Care" o "Transitional Care", "Child" y "Enfermedad crónica". Los criterios de inclusión fueron artículos publicados entre 2009 y 2018, en portugués, inglés y español. Si bien los criterios de exclusión fueron artículos no disponibles en línea en formato completo, tesis y disertaciones, publicaciones duplicadas y estudios que no abordaron el tema. Se seleccionaron diecinueve artículos en la muestra final, construyendo las categorías: Comunicación entre Servicios de la Red de Salud; Transición de la atención del hospital al hogar; Rehospitalizaciones y su relación con la continuidad y transición asistencial; y, Enlace longitudinal y continuidad relacional. Entre los hallazgos, la evidencia trae aportes prácticos al área de la salud, especialmente para enfermeras profesionales, ya que presenta estrategias para el cuidado de niños con condiciones crónicas de salud, además de señalar las dificultades vividas en este contexto, destacando aspectos que necesitan ser mejorados.
