Fuerza de los músculos inspiratorios y función pulmonar en pacientes con insuficiencia cardíaca crónica y apnea obstructiva del sueño

Resumen

Objetivo: Evaluar la fuerza de los músculos inspiratorios y la función pulmonar en pacientes con insuficiencia cardíaca crónica (ICC) y apnea obstructiva del sueño (AOS). Métodos: Estudio transversal con 18 participantes con ICC (12 hombres) presentaron fracción de eyección del ventrículo izquierdo (FEVI) reducida (< 51% para los hombres y < 53% para las mujeres), afectado por AOS (leve a grave) diagnosticado mediante polisomnografía portátil. La fuerza muscular inspiratoria fue medida por la determinación de la presión inspiratoria máxima (Pimáx) por manovacuometría y comparado con los valores predichos para el sexo y la edad. La capacidad vital forzada (CVF), el volumen espiratorio forzado en el primer segundo (VEF1)   y el índice de tiffenau (CVF/VEF1) se midieron utilizando un espirómetro computarizado y se compararon con los valores predichos.  Resultados: Se preservó la fuerza muscular inspiratoria de los pacientes con ICC y AOS (109 ± 29 cmH2O) en comparación con los valores predichos para sexo y edad (100 ± 13cmH2O; p= 0,22). Tanto la CVF como la VEF1 fueron disminuidas en relación con los valores predichos (p= 0,001). Conclusiones: Los pacientes con ICC y AOS presentaron fuerza muscular inspiratoria preservada y disminución de la función pulmonar con relación a los valores de referencia.
