Implicaciones materno-fetales y neonatales del lupus eritematoso sistémico durante el embarazo: una revisión integradora

El lupus eritematoso sistémico (LES), una condición autoinmune que causa daño renal, cutáneo, neurológico, hematológico, cardiológico y pulmonar, presenta un patrón clínico heterogéneo con mayor frecuencia en el sexo femenino. Las mujeres en edad fértil se ven afectadas en mayor proporción, causando daño por la agudización de la enfermedad a órganos fundamentales para el desarrollo fetal y la supervivencia materna. En ese sentido, la presente revisión tuvo como objetivo comprender las posibles relaciones entre el lupus eritematoso sistémico, el embarazo y el período neonatal. Para la investigación se utilizaron las bases de datos SciELO y PubMed (Medline). Posteriormente, se aplicaron los criterios de elegibilidad: ensayos clínicos aleatorizados; ensayos clínicos; metanálisis y artículos publicados en los últimos 10 años con texto completo disponible en inglés y portugués. De 113 artículos buscados y analizados, 26 artículos se incluyeron para esta revisión integrativa de la literatura. En estos estudios, algunas situaciones como actividad lúpica actual o menos de 6 meses antes de la concepción, afectación renal, pulmonar o cardiaca, suponen un gran riesgo materno y/o fetal, por lo que se consideran desaconsejadas durante el embarazo.. En este sentido, se recomienda la consejería preconcepcional para mujeres con LES con deseo de concebir. Así, el embarazo de las mujeres con lupus se ha convertido en un proceso cada vez más seguro con el advenimiento de terapias más efectivas para el tratamiento de la enfermedad.
