Coronectomía: una revisión narrativa de la literatura

Resumen

La extracción quirúrgica de terceros molares mandibulares impactados cuyas raíces dentales están cerca del canal del nervio alveolar inferior (NAI) representa un procedimiento de alto riesgo para el desarrollo de lesiones en esta estructura anatómica. La lesión de IAN es una complicación rara, a menudo grave, que puede ocurrir como resultado de la extracción del tercer molar mandibular. Su incidencia es viable del 0,5% al ​​8%, pero puede aumentar del 20 al 50% cuando las raíces contactan con la cortical ósea del nervio dentario inferior.La técnica de coronectomía se ha propuesto como alternativa a la extirpación completa de un tercer molar inferior impactado ubicado junto al NAI. La técnica consiste en extraer únicamente la corona del diente, dejando la raíz en el alvéolo, evitando teóricamente daños directos o indirectos en el NAI. Este trabajo se propone realizar una revisión narrativa de la literatura, buscando discutir la efectividad de la coronectomía en relación a los trastornos sensoriales, al remover terceros molares inferiores impactados y posicionados desfavorablemente en relación al nervio alveolar inferior. Las evidencias sobre los resultados a largo plazo de la coronectomía permiten afirmar que es un método seguro y eficaz en los casos en que el tercer molar inferior se encuentra en una posición de alto riesgo de complicaciones que involucran el NIA.
