Membrana plasmática rica en fibrina alrededor de los implantes osteointegrados para la fijación de una prótesis tipo sobredentadura: ensayo clínico aleatorizado, boca partida, doble-cego

Resumen

Los avances en el área de la cirugía oral han mejorado el pronóstico de los implantes osteointegrados debido al desarrollo de biomateriales, como la membrana plasmática rica en fibrina (PRF), que pueden mejorar el proceso de regeneración y  cicatrización. Este trabajo tiene como objetivo comparar las técnicas de injerto conectivo subepitelial (TC) y el uso de membranas PRF, evaluando su capacidad real para curar y mantener los tejidos alrededor de los implantes osteointegrados. En la muestra con 8 pacientes completamente desdentados, cada individuo recibió 02 implantes para la fijación de una prótesis tipo sobredentadura, uno de los cuales recibió un procedimiento quirúrgico regenerativo gingival (grupo TC) y el otro implante recibió la membrana plasmática rica en fibrina (grupo PRF) . Se evaluaron los siguientes parámetros periimplantarios: profundidad de sondaje, ancho de la mucosa queratinizada y cociente de estabilidad del implante (ISQ). Comparando las medidas profundidad de sondaje del grupo TC (2,21mm±0,58) y el grupo PRF (2,21mm±0,52) y ancho de la mucosa queratinizada del grupo TC (2,17mm±0,67) y el grupo PRF (2,29mm±0,45) entre el tiempo inicial (T0) y después de 3 a 5 meses (T1), se observó que no hubo diferencias estadísticas (p>0.05). No hubo diferencia estadística en Unidad de Estabilidad entre el grupo TC (58,93±14,98) y el grupo PRF (62,58±10,51) (p >0,60). Análisis realizados en el software XLSTat versión 2017 (Addinsoft, 2017), considerando un nivel de significancia del 5%. El uso de PRF en cirugías periimplantarias se puede considerar como una opción clínica para mantener la salud periimplantaria.
