Toxina botulínica en el tratamiento de pacientes con disfunción temporomandibular y dolor de cabeza: Una revisión del alcance

Resumen

La disfunción temporomandibular (DTM) y los dolores de cabeza son problemas de salud desafiantes y representan la segunda causa de dolor facial crónico en la población mundial. Presentan convergencia en el tratamiento, siendo la toxina botulínica un recurso de segunda línea para los casos refractarios. Con el objetivo de mapear información sobre el tema, se realizó esta revisión de alcance utilizando los elementos del acrónimo PCC para formular la pregunta: “¿Qué se ha producido en la literatura sobre el uso de la toxina botulínica A en pacientes con TMD y cefalea refractaria? a la terapia inicial?”. La población fue definida por pacientes con DTM y cefaleas; en el concepto, se utilizó la toxina botulínica A en el contexto de pacientes refractarios al tratamiento. Las bases de datos utilizadas fueron: Pubmed, LILACS, Cochrane y literatura gris. Dos investigadores, a ciegas, utilizaron el administrador de referencias Rayyan para juzgar los criterios de inclusión y exclusión de los artículos. Se incluyeron diseños de estudio variados, sin restricciones de idioma o año de publicación. Los desacuerdos fueron resueltos por un tercer evaluador. Los artículos seleccionados para lectura pasaron a la extracción de datos. Los estudios no presentaron uniformidad en sus diseños y no hubo consenso sobre los resultados. La mejoría del dolor se observó en más del 80% de los estudios, mientras que el aumento de la apertura bucal fue el dato que presentó resultados más controvertidos. El análisis de la calidad de vida seguía siendo una laguna inexplorada.
