Uso de inibidores da aromatase para melhora estatural em meninos: uma revisão literária

Introdução: Baixa estatura, atualmente, é uma das causas mais comuns de procura e encaminhamento de pacientes para a endocrinologia pediátrica. Tal procura se dá, não somente devido aos fatores fisiológicos, mas também psicológicos envolvidos. Objetivo: analisar a literatura específica sobre os inibidores da aromatase e seu uso em crianças. Método: Foi realizada uma revisão narrativa de literatura, utilizando as bases de dados Pubmed, Google acadêmico e UptoDate. Para a busca, foram utilizados os descritores: “aromatase inibithor”, “children” e “growth”. Foram selecionados artigos sobre o uso dos inibidores da aromatase em meninos com baixa estatura. Excluíram-se artigos que se voltavam para uso de inibidores da aromatase em câncer, uso dos inibidores da aromatase exclusivamente em Síndrome McCune Albright e que não estavam localizados na íntegra. Foram encontradas 1028 produções cientificas, das quais 59 correspondiam aos critérios pré-estabelecidos. Resultados e Discussão: A enzima aromatase é uma enzima do citocromo P450 e faz a conversão de precursores androgênicos em precursores estrogênicos. Os inibidores da aromatase retardam a senescência da cartilagem de crescimento, podem ser indicados em meninos com baixa estatura atual com repercussão psicossocial ou previsão inadequada da estatura final. Segundo estudos previamente publicados, os inibidores da aromatase foram eficazes no tratamento de baixa estatura. Conclui-se que o tratamento com inibidor da aromatase efetivamente atrasa a maturação óssea e melhora a altura adulta.
