Tratamiento de endodoncia en una sola sesión en un paciente con necesidades especiales bajo anestesia general: Reporte de caso

Los pacientes con necesidades especiales suelen estar a la defensiva o muestran una ansiedad extrema durante el cuidado dental convencional, requiriendo tratamiento en el hospital, bajo anestesia general. Un paciente diagnosticado con autismo fue atendido en la CAOE (FOA/UNESP). En la evaluación clínica y radiográfica, se detectó la necesidad de procedimientos dentales, incluido el tratamiento endodóntico. Por falta de cooperación, el paciente fue derivado al hospital Santa Casa de Misericórdia. Este informe describe la endodoncia en una sola sesión de tres elementos bajo anestesia general. Tras la sedación se realizó apertura coronaria, seguida de aislamiento absoluto. La preparación biomecánica de los conductos radiculares se realizó mediante una técnica mixta: preparación cervical/media con fresas Gates-Glidden y limas manuales para preparación apical, bajo irrigación con hipoclorito de sodio (2,5%). El diente real y la longitud de trabajo se obtuvieron con la ayuda del localizador foraminal Root ZX II. Después de utilizar EDTA (17%), nuevo riego con hipoclorito de sodio y secar los canales, se realizó el llenado. Se utilizó cemento a base de hidróxido de calcio Sealapex y condensación lateral, seguida de restauración con resina compuesta. El tratamiento de endodoncia, aún con las complejidades de estas condiciones, está indicado para evitar extracciones, ya que estos pacientes presentan pérdida precoz por mala higiene bucal por dificultades motoras. La tasa de supervivencia de los dientes tratados endodónticamente bajo anestesia general fue favorable en la literatura, así como los reportes de mejoría en la calidad de vida de pacientes con necesidades especiales.
