Tratamento endodôntico em sessão única em paciente portador de necessidade especial sob anestesia geral: Relato de caso

Resumo

Pacientes portadores de necessidades especiais costumam ser defensivos ou demonstram extrema ansiedade durante atendimento odontológico convencional, necessitando de tratamento em âmbito hospitalar, sob anestesia geral. Paciente diagnosticado com autismo foi atendido no ambulatório do CAOE (FOA/UNESP). Na avaliação clínica e radiográfica foi detectado necessidade de procedimentos odontológicos, dente eles o tratamento endodôntico. Devido à não cooperação, o paciente foi encaminhado ao hospital Santa Casa de Misericórdia. Este relato descreve endodontia em sessão única de três elementos sob anestesia geral. Após sedação, foi realizada abertura coronária dos 3 elementos, seguido de isolamento absoluto. O preparo biomecânico dos canais radiculares foi realizado utilizando técnica mista: preparo cervical/médio com brocas Gates-Glidden e limas manuais para preparo apical, sob irrigação de hipoclorito de sódio 2,5%. O comprimento real do dente e de trabalho foi obtido com auxílio de localizador foraminal Root ZX II. Após uso de EDTA 17%, nova irrigação com hipoclorito de sódio e secagem dos canais, procedeu-se para a obturação. Foi utilizado o cimento à base de hidróxido de cálcio Sealapex e uso da condensação lateral, seguido da restauração definitiva com resina composta. O tratamento endodôntico, mesmo com as complexidades dessas condições é indicado para evitar extrações, uma vez que esses pacientes apresentam perda precoce por deficiente higienização bucal devido à dificuldade motora. A taxa de sobrevida dos dentes tratados endodonticamente sob anestesia geral se mostrou favorável na literatura, assim como relatos da melhora na qualidade de vida dos pacientes portadores de necessidades especiais.
