Infecciones fúngicas invasivas em receptores de transplante renal y hepático em um centro del nordeste de Brasil

Resumen

Objetivo: Describir las principales infecciones fúngicas invasivas (IFI) tras el trasplante renal y hepático en un centro de referencia, así como su evolución, tratamiento y características clínicas. Material y métodos: Se trata de un estudio retrospectivo, observacional, descriptivo, de serie de casos que involucró a IFI diagnosticadas entre enero de 2012 y diciembre de 2019 en receptores de trasplante de riñón e hígado. Resultados: Entre 769 trasplantes de riñón, solo 1 paciente recibió el órgano de un donante vivo y los otros trasplantes fueron de donantes fallecidos. Se diagnosticaron 15 IFI (7 histoplasmosis, 4 criptococosis, 3 candidemias y 1 aspergilosis), mientras que en 673 receptores de trasplante hepático se diagnosticaron 8 IFI (6 candidemias, 1 murcomicosis y 1 criptococosis). Del total de 23 pacientes, 6 (26%) tenían una infección diagnosticada dentro de los 6 meses posteriores al trasplante. El régimen inmunosupresor primario utilizado fue tacrolimus (82,6%), prednisona (82,6%) y micofenolato (56,5%). El anfotericina B desoxicolato fue el principal antifúngico utilizado en el tratamiento, con nefrotoxicidad en el 80% de los casos. En el seguimiento clínico, 14 pacientes progresaron a curación (60,9%) y 9 a muerte (39,1%). En la mayoría de los pacientes del presente estudio se observó un empeoramiento de la función renal. Conclusión: Candidemia, histoplasmosis y criptococosis fueron las IFI más frecuentes, la mayoría ocurrieron más tarde, 6 meses después del trasplante, y se asociaron con alta mortalidad.
