Calidad de capa híbrida de adhesivos universales: una revisión sistemática de la literatura

Resumen

Actualmente, la disponibilidad de una amplia variedad de adhesivos universales dificulta que los dentistas elijan el sistema correcto para diferentes situaciones de unión al sustrato de dentina. Este trabajo presenta una revisión sistemática que evaluó la calidad de la capa híbrida creada por las diferentes estrategias de aplicación de los sistemas adhesivos universales, en la modalidad de autograbado y en la modalidad de acondicionamiento y lavado, con el objetivo de investigar si las diferencias observadas en la capa híbrida son importantes para la práctica clínica. Así, dos revisores realizaron una búsqueda bibliográfica hasta diciembre de 2022 en la base de datos electrónica PubMed (MedLine). Solo se incluyeron estudios in vitro que evaluaron la calidad de la capa híbrida mediante microscopía confocal de barrido láser y que informaron el efecto del uso de adhesivos universales en la calidad de la capa híbrida de dentina. Se encontró que la forma de aplicar los adhesivos universales influye en la calidad de la capa híbrida. Los adhesivos aplicados en la modalidad de grabado y lavado presentan la capa híbrida más gruesa y con mayores extensiones en los túbulos dentinarios. Sin embargo, los adhesivos autograbadores demuestran una mayor longevidad debido a que su interfaz resina-dentina es más resistente al envejecimiento y los estudios muestran que un enfoque de grabado antes de la aplicación del sistema adhesivo no mejora la resistencia de unión. Con base en estos resultados, podemos concluir que el enfoque autograbador es la opción más ventajosa para aplicar en dentina, ya que brinda mayor longevidad y menor posibilidad de nanofiltración y degradación de la capa híbrida con el envejecimiento.
