Aplicación del Score de Hueso Trabecular para la evaluación del riesgo de fractura por fragilidad em mujeres com Diabetes mellitus tipo 2

Resumen

Objetivo: Evaluar el Score de Hueso Trabecular y el riesgo de fracturas mayores y de cadera mediante el FRAX® Brasil ajustado por el TBS en mujeres con Diabetes Mellitus tipo 2 (DM2) posmenopáusicas. Materiales y Métodos: Estudio transversal realizado en el sector de densitometría ósea (DXA) de una clínica privada. Fueron elegibles para el estudio 124 mujeres sometidas a DXA y score de hueso trabecular (TBS). Estas fueron agrupadas en el Grupo Control (GC) sin DM2 y el Grupo Diabetes (GD) con DM2, emparejadas por edad, peso y altura. Resultados: Los factores clínicos que presentaron mayor frecuencia fueron osteoporosis secundaria y fractura previa. Entre las fracturas, la del antebrazo tuvo mayor frecuencia. El promedio del TBS fue mayor en el Grupo Control: GC = 1,338 ± 0,08 vs GD 1,279 ± 0,1 p <0,002. El FRAX® Brasil ajustado por el TBS detectó mayor riesgo de fracturas en el Grupo Diabetes. Hubo una correlación negativa entre la edad y el TBS en ambos grupos (r = -0,25 p < 0,004). Conclusión: El uso del TBS y del FRAX® Brasil ajustado por el TBS en mujeres posmenopáusicas con DM2 fue capaz de identificar un menor TBS y un mayor riesgo de fracturas, incluso en individuos sin diagnóstico densitométrico de osteoporosis.
