Relação entre a carga de trabalho e sintomas no teste ergométrico em pacientes com doenças pulmonares intersticiais e em indivíduos saudáveis

Resumo

Introdução: As doenças pulmonares intersticiais (DPI) apresentam características clínicas semelhantes caracterizadas por dispneia e intolerância ao exercício. Essas características se acentuam durante o esforço físico, embora isso ainda não tenha sido investigado em profundidade nessa população. Objetivos: Comparar a sensação subjetiva de dispneia e fadiga por carga de trabalho durante o teste ergométrico (TE) entre pacientes com DPI e indivíduos saudáveis. Métodos: Os indivíduos realizaram o TE em cicloergômetro com protocolo incremental. A cada estágio do teste foram coletados a sensação de esforço pela escala de Borg de dispneia (D) e fadiga (F). As inclinações das curvas de D e F por estágio no teste foram comparadas por meio de regressão linear utilizando o método Zar (teste F). Resultados: Foram estudados 37 pacientes com DPI e 29 controles. O grupo DPI apresentou menor carga máxima (60[40-80] vs saudáveis 120[100-160]; (P<.0001) e maiores sintomas ao final do teste para D (P=0,02), mas não para F (P=0,23) quando comparados ao controle. Ainda, o grupo DPI apresentou mais sintomas por watt (i.e. Borg/Wattts) no TE comparado ao grupo controle para D (8x10-2±7x10-2 vs 2x10-2 ± 2x10-2; P=0,0007) e F (1x10-1± 9x10-2 vs 3x10-2 ± 1x10-2; P=0,001). A inclinação das curvas Borg/watts diferiu entre os grupos, com DPI apresentando mais sintomas de D e F por watt (P<0.05 entre as curvas). Conclusão: Pacientes com DPI apresentarem menor capacidade máxima de exercício e parecem apresentar um aumento mais acentuado na sensação de D e F por carga de trabalho durante o TE.
