Relação entre a sobrevida no câncer de cólon, linfonodos ressecados e as características clínico-demográficas

Resumo

Objetivo: Estabelecer a relação entre a sobrevida do câncer de cólon, linfonodos ressecados e suas características clínico-demográficas. Metodologia: Trata-se de um estudo observacional, retrospectivo, descritivo e transversal, baseado na revisão de prontuários médicos. Resultados: O estudo revisou 230 prontuários de pacientes com câncer colorretal (CCR), sendo mais da metade mulheres entre 50 e 75 anos. 71,3% das cirurgias foram eletivas, com 65,2% realizadas por videolaparoscopia, sendo a localização mais comum o cólon retossigmoide em 6 de 10 pacientes. Pacientes acima de 50 anos tiveram maior sobrevida mediana de 96 meses. A sobrevida foi maior para tumores no cólon transverso, N0 e estágio IIB. A maioria dos tumores foi classificada como T3 (54,8%) e N0 (53,9%). 90% dos pacientes estavam em M0. A sobrevida foi maior em pacientes com 12 ou mais linfonodos removidos (88 meses) em comparação com menos de 12 (80 meses). 89,6% não receberam terapia neoadjuvante, mas entre os que receberam, 9,1% usaram quimioterapia e radioterapia. A quimioterapia foi o tratamento adjuvante em 52,6% dos casos. Conclusão: O CCR permanece um problema de saúde, com aumento da incidência em pacientes mais jovens. A sobrevida global é influenciada por fatores como localização do tumor, sexo, idade, estágio da doença e envolvimento linfonodal. No estudo, a remoção de 12 ou mais linfonodos não trouxe vantagem significativa na sobrevida, e a presença de linfonodos metastáticos piorou o prognóstico. A linfadenectomia continua sendo fundamental no tratamento curativo do CCR, permitindo um melhor estadiamento e orientação para adjuvância.
